Across the country, leaders from various systems and disciplines are talking about individuals who are frequently coming through their system, with recurrent admissions to emergency departments and jails. This results in costly and often inappropriate care with little to no change in outcomes. Using data to understand the reasons individuals are frequently circulating through systems and to chart a different course may not be as intuitive as you might assume. Generally, successful strategies require multidisciplinary systemic coordination that pushes each system to be accountable for the services they provide and shared responsibility for the outcomes.

In this webinar, participants will learn how to stratify justice system populations and build an integrated, strategic framework to successfully address the needs of persons often referred to as “familiar faces” or high utilizers, of justice, primary health, and behavioral health systems. Participants will also learn about familiar face strategies, how to use data to identify and sort populations, and how to create an evaluation process that guides and informs the work.
